To make your visit to AFC Urgent Care go as smoothly as possible, please have the following information ready for your visit.
- Insurance Card – Click here to see what types of insurance we accept. Insurance is not required to visit our walk in clinic.
- Photo Identification – All adults patients need to provide a valid photo ID. If you are seeking treatment for a child a parent or guardian must provide their photo ID.
- Co-Pay – We accept cash, checks (with ID) and all major credit cards.
The Walk-In Clinic Process
Check In – Upon arrival to AFC Urgent Care, first time patients will be asked to complete a short registration form. To speed up the process, you can register online right now.
Medical Exam – After check-in, you will be brought into a “triage” room where our trained staff will assess your needs. You will then be seen by a professional medical provider who will treat your illness or injury.
